Siouxland Scanner has received information that USBANK filled out a withdraw slip for a customer at a Sioux City Branch, they accidently used the wrong account information (for someone with the same name in Omaha). The customer signed the withdraw slip as usual. Then when the account owner realized the unauthorized withdrawal it was reported it as forgery/theft to Police and security camera footage released before doing research into what happened.
